This standardized test is reproducible and validated in research and clinically, and is the gold standard for diagnosing sleep apnea. Nocturnal polysomnography is the most comprehensive examination and remains the preferred choice for evaluating sleep-related breathing disorders. However, it requires a technician and must be performed in a laboratory or a sleep specialized clinic using heavy equipment. PSG consists of a night of recordings of various parameters for a duration of at least 6 hours. This comprehensive test allows for the diagnosis of several types of sleep disorders, including obstructive sleep apnea syndrome (OSAS), restless legs syndrome, and narcolepsy."

This is the most common type of sleep apnea. • Other factors that can contribute to obstructive sleep apnea include the presence of: • enlarged tonsils obstructing the airway, • excess fatty tissue in the throat in obese individuals, • an overly long soft palate, • a lower jaw that is too short or set back (mandibular retrognathia). • Obstructive apnea requires respiratory effort to overcome an obstruction in the upper airway (nose, mouth, pharynx, and larynx). • Obstruction can vary in severity and may be due to age (aging), malocclusion, hormonal imbalance, diabetes, enlarged adenoid tissues (tonsils, adenoids), or excess weight."

The measurement of blood oxygen saturation indicates the percentage of oxygen-carrying hemoglobin in the blood. The device is worn for one night, and the data is recorded in the device to be subsequently interpreted by computer. This test can be performed in a \"sleep lab,\" which is a clinic where one can spend the night. Some partial tests can be done using a portable device that the patient can use at home for home monitoring. This allows for the evaluation of oxygen levels, airflow through the nose and mouth, respiratory variations, and snoring. Diagnosis is based on the presence of an excessive number of events (apneas and hypopneas) observed during the polysomnographic recording. This test can measure the following: • brain activity • sleep stages (duration, awakenings, etc.) • eye movements • muscle tone • pulse • leg movements • respiration • airflow • chest and abdominal movements • blood oxygen level."

It corresponds to a decrease in blood oxygen saturation of at least 3-4% on a polysomnography, but there is no consensus on the duration of this event. However, many accept a duration of 10 seconds as a reference for hypopnea, just as for apnea. Educational information. A diagnosis of sleep apnea must be made by a physician, following a sleep evaluation.

Quick answer

## What is Apnée Laval?

Apnée Laval is a dental sleep medicine clinic in Laval, Quebec. The team assesses snoring and sleep-related breathing concerns and makes custom mandibular advancement devices, working with the patient’s physician.

- Services are offered in French and English.
- A sleep apnea diagnosis is made by a physician, after a sleep evaluation.
- The appliance is made from impressions of your mouth and adjusted during follow-up.

Do you recognize yourself?

## Does this sound like you?

These signs are not a diagnosis, but they often warrant a discussion with a professional.

### Loud, regular snoring

Snoring that disturbs those around you can be a sign of a sleep-related breathing disorder.

### Observed breathing pauses

Your partner notices pauses in breathing or loud gasps for air during the night.

### Fatigue upon waking

You sleep enough hours, but wake up without feeling rested.

### Daytime sleepiness

You struggle to stay awake at work, in meetings or while driving.

### Morning headaches

Headaches upon waking or a dry mouth happen regularly.

### Difficulty with CPAP

You have a machine, but can’t manage to use it through the whole night.

Understanding

## What happens in your airway

A simple illustration to visualize the role of jaw position during sleep.

![Illustration: mandibular advancement device in place, airway more open](/__l5e/assets-v1/2ff5f4a6-a1c7-47c6-b814-b99087a2b371/airway-after-v2.jpg)

![Illustration: relaxed jaw during sleep with reduced airway space](/__l5e/assets-v1/2a916b25-68a9-4659-b95d-23e5a379063a/airway-before-v2.jpg)

Without appliance With appliance

### Without an appliance the jaw falls back

During sleep, muscles relax and the jaw slides backward. The airway narrows, which can cause snoring and pauses in breathing.

### With the appliance the airway stays open

The custom appliance gently holds the jaw forward. The airway stays clear and air flows more freely through the night.

### Custom-made, discreet and portable

Made from impressions of your mouth, the appliance gently holds the jaw forward during sleep to keep the airway open.

No electricity, no mask and no noise: it fits in a case the size of your fist.
